A Smooth, Supportive Start for You and Your Family

Beginning a new chapter in skilled nursing care can feel overwhelming, especially when a transition is happening quickly from a hospital stay, emergency room visit, or assisted living community. At Country Health, we are happy to help you every step of the way.

In most cases, our admissions team works directly with hospital discharge planners and social work departments to gather required admission paperwork on your behalf. We coordinate the details, communicate with care teams, and help ensure a smooth, well-supported transition.

What to Bring When You're Ready to Begin

For many admissions, much of the required documentation is already provided through the hospital or referring facility. When additional information is needed, our team will be here to guide you.

You may be asked to provide or confirm the following items:

  • Social Security Card 
  • Insurance Cards 
  • Medicare and/or Medicaid Cards 
  • Power of Attorney Documentation (both healthcare & financial)

  • Advance Healthcare Directives (Living Will, POLST forms, etc.)

If you’re unsure what’s needed, or don’t have everything readily available, don’t worry. Our admissions team will walk you through what’s required and help coordinate any missing pieces.

What Happens Next

Once we’ve connected with the referring facility and gathered the necessary information, our admissions team takes it from there. We’ll work closely with you and your care team to:

  • Coordinate admission details and finalize any remaining paperwork
  • Communicate with discharge planning and social work teams
  • Review services, care options, and daily life at Country Health
  • Answer questions about care, therapy, activities, and amenities
  • Confirm a move-in timeline that aligns with your discharge plans
